Why do I need to sign a consent form before the session?

A health questionnaire and a signature are required before every session: they protect your health and provide a legal framework for the service.

Before every session, your artist will have you fill out a short health questionnaire and ask for a signature. This is a required step that protects your health and provides a legal framework for the service, just like you'd expect from a healthcare professional.

What this form covers

The form is usually filled out right at the studio, on a tablet or touchscreen, just before starting. It includes a few parts.

  • A health questionnaire: a few simple questions about your general health (allergies, current treatments, pregnancy, skin conditions...), so your artist can adapt the session if needed.
  • A handwritten signature: you sign directly on the screen, with your finger or a stylus.
  • A statement: you confirm you've answered the questionnaire honestly.
  • An optional media release: if you agree, your artist may ask permission to share a photo of your tattoo, for example on their Instagram or portfolio. This box is always optional, it's entirely your choice.

Why it's required

This document serves two purposes.

  • Protecting your health: certain information (allergies, treatments, pregnancy) may require adapting or postponing the session. It's better to flag it beforehand than after.
  • Providing a legal framework for the service: your signature formalizes your informed consent to receive a tattoo, based on the information you've provided yourself.

Without this form filled out and signed, your artist cannot start the session.

Answering the questionnaire honestly

Take this questionnaire seriously, even if it seems quick to fill out. If you're unsure about a piece of health information (a recent treatment, a skin condition, pregnancy), mention it to your artist before answering: it's not up to you alone to judge whether something matters.

When you sign

This form is tied to each individual appointment: you'll sign it again if you come back for another session, even with the same artist and to continue a project already underway.

After signing
